Program Overview
The Registered Nurses and Registered Practical Nurses of the Primary Care Team offer chronic disease management and health promotion programs in the doctor’s offices of the Algonquin Family Health Team.
The programs provided in your doctor’s office could include:
Well Baby/Well Child visit: growth and development assessment and immunization based on Rourke screening tool, NACI and Ontario Public Health guidelines
18 month Well Baby exam: an enhanced Well Baby visit including the Nippissing District Developmental screen, Rourke screening tool, growth and development assessment and immunization based on NACI and Ontario Public Health guidelines
Cancer Screening: education and promotion re: mammogram, pap smear and fecal occult blood test based on Cancer Care Ontario guidelines
Medication Review: one-on-one medication review for patients taking 4 or more medications
Prenatal and Postnatal care: pre and postnatal screening, education and health promotion per Public Health Agency of Canada
Diabetes Management: assessment, education and health promotion based on Canadian Diabetes guidelines
Hypertension Management: assessment, education and health promotion based on Canadian Hypertension Education Program
Adult Immunization: assessment, education and health promotion based on NACI and Ontario Public Health guidelines
Registration Information
People who are patients of the Algonquin FHT can refer themselves to the Primary Care Nurse in their doctor’s office by contacting their family doctor and booking an appointment. For your convenience, your appointment can be booked just prior to your doctor’s appointment.
