The Algonquin Family Health Team (AFHT) is a non-profit, community-wide organization that provides comprehensive primary health care in Huntsville, Dorset,  and the surrounding areas. AFHT is committed to protecting the privacy of our patients’ personal health information, as well as the personal information of our employees.

In order to provide health services to you and plan your care, AFHT needs to collect, use and disclose personal health information about you.  You have a right to expect that we will use all reasonable means to keep your personal health information accurate, confidential and secure.  AFHT is committed to a high standard of privacy for its information practices.  AFHT complies with applicable privacy legislation, namely, the Personal Health Information Protection Act (PHIPA).
