Our Mission

Our mission is to preserve hope and promote quality of life for people living with a life threatening illness.

The Palliative Care Team supports individuals and their families in Huntsville and surrounding area cope with the physical, emotional, social, psychological and spiritual concerns caused by living with a life threatening illness.

What is Palliative Care?

Palliative care is active, total care for those facing a life threatening illness. It involves an interdisciplinary team of health care professionals who provide coordinated services for individuals and their families. Our team consists of Nurse Consultants, Social Worker, Physician Consultants and Pharmacy Support.

Referrals

We accept referrals from a variety of sources including patients, family/friends, physicians, nurses, community agencies, hospices, and long term care homes. You can access and print our referral form here.
